About This Course
Reverse Failure Mode and Effects Analysis (RFMEA) is a structured, shopfloor-based verification method used to confirm that the Process FMEA (PFMEA) reflects actual production conditions.
Unlike traditional PFMEA, which is a planning and risk anticipation tool, RFMEA is a verification and validation tool performed on the live process.
RFMEA answers three critical questions: 1. Does the real process match the PFMEA documentation? 2. Are the identified controls actually effective? 3. Are there undocumented risks in the process? RFMEA is increasingly required by OEMs as part of: • IATF 16949 compliance (risk-based thinking and process effectiveness) • AIAG–VDA FMEA Handbook alignment • Customer Specific Requirements (CSR) • Manufacturing risk validation programs RFMEA strengthens: • Process robustness • Control plan effectiveness • Operator understanding • Audit readiness • Customer confidence It transforms FMEA from a “document exercise” into a living risk management system.

Prerequisites for Attendance
Participants should have: • Basic understanding of PFMEA methodology • Familiarity with Control Plans • Understanding of process flow diagrams • Knowledge of Severity (S), Occurrence (O), Detection (D) and Action Priority (AP)
Grading and Certification
To earn the certification for completing and passing the ID40410 Reverse Failure Mode and Effects Analysis (RFMEA) course, there will be a Knowledge Test at the end of the course that must be passed with a minimum score of 60% within the 20-minute time limit. The score for this assessment is the only graded activity that will be considered as part of the final score for this course. Detailed instructions are provided prior to the knowledge test, which you will be able to see once you get to this part of the course. .
